How International Patients Use Shanghai’s International-Care Policy

Shanghai treats international care as a checkable local standard, not a nationwide package. Circular Hu Fu Ban Fa〔2024〕32 requires providers to follow DB31/T 1487—2024: multilingual records, multi-channel payment, and foreign-language booking. Overseas-approved clinically urgent products still need an application under the rules. They are not automatic.

What the implementation opinions cover

The General Office of the Shanghai Municipal People’s Government issued the Implementation Opinions on 30 December 2024 (published 8 January 2025). The service system covers outpatient and emergency care, inpatient stays, vaccination, checkup, health management, and TCM, and encourages internet and remote care. Competitiveness clauses support specialty-disease brands and encourage CAR-T and particle therapy, plus clinical use of innovative technologies and products.

On openness and payment, the circular implements MOFCOM / NHC / NMPA healthcare opening pilots, including wholly foreign-owned hospital pilots; seeks support for international-care institutions to apply, as prescribed, to use overseas-marketed clinically urgent drugs and devices; and encourages international commercial-insurance accreditation. Facilitation includes easier use of foreign travel documents at the point of care, foreign-language booking platforms, cash / foreign cards / mobile pay / insurer direct-pay, FX accounts, multilingual records, and a clinical coordination mechanism.

Visit growth is not your slot

On 27 February 2026, shanghai.gov.cn reprinted a Jiefang Daily report: Shanghai Health Commission figures show public hospitals served 73,200 foreign-passport visits in 2025, versus 67,600 in 2024 (over 8% growth). The report calls DB31/T 1487—2024 the first national-level international-care service standard; Huashan Hospital, the lead drafter, saw international-care outpatient visits rise 25% year-on-year in January–November 2025. Volume is not a reserved bed.

Go to the booked floor, not the general queue

Encouraging CAR-T or particle therapy does not mean every hospital or every indication. Quality-based pricing and insurer direct-pay depend on tax and price rules and a contract with the insurer. Foreign-language pages, direct-pay, and overseas-product applications follow that hospital’s current written capacity.
